1. The following documents must be submitted with the application.

Certificate of Company Registration

Permanent Account or VAT Registration Certificate

Tax Payment Certificate (A.Y. 2081/082)

Authorized Seller Certificate (only if applicable)

Registration certificate from the Department of Drug Management (only in the case of drugs)

2. You must submit a quotation for the goods and services you wish to distribute within the time of registration.

3. The hospital will enter into a separate purchase agreement with the firm to be listed.

4. Applications that are not received within the specified time and are incomplete will not be accepted.
