Notice Regarding Vendor Enlistment

1. Purchase and repair of blood transfusion related blood bags, reagents, and other lab supplies and laboratory equipment.

2, Printing (frames, love symbols, flags) and stationery, hotel services, vehicle spare parts and repairs, computers, printers, furniture and furnishings, purchase and repair of electrical equipment,

3, Disaster relief materials - food and non-food items (tents, blankets, mats, cooking utensils) and food items.

4, Pipes, fittings, wire mesh, rods, cement, improved iron stoves, logo plates related to water supply and sanitation projects,

5. All materials including transportation and vehicles.

6. Regular office cleaning supplies.

Required documents to be submitted with the application:

1) Copy of firm, company registration, permanent account number, value added tax registration certificate,

2) Copy of tax payment certificate up to fiscal year 2081.082.

3) Rates of services, facilities and materials to be provided.

1) Applications received after the deadline or without the required formalities will not be registered.

2) Listed companies and firms may be removed from the list if they do not provide satisfactory services and goods. No claim can be made against this.

3) You should apply on your firm's letterhead, mentioning the group and work you want to do, and addressing it to this office, along with your address, contact number, email and fax number, and writing 'Application for Registration of Lists for the Fiscal Year 2083.084' on the outside of the envelope.

4) You must clearly state the group for which you are applying to list and the activities that fall under that material. You must also attach a document that authorizes you to perform that activity.

5) You are requested to submit a statement of which companies and at what rates you can provide services and materials.
