4. Copy of tax payment certificate for FY 2081182,

5. Copy of professional license certificate (only in cases where a license is required),

6. A self-declaration application stating that one is not disqualified or blacklisted under the prevailing law,

7. Certified copy of the register of shareholders and debenture holders as per sub-section (1) of Section 46 of the Companies Act, 2063 (only in the case of organizations registered under the Companies Act).
