Notice regarding provision of vehicles for rent for collection and transportation of non-biodegradable waste

The Office of the Phedikhola Rural Municipality, Village Executive has decided to rent a 4WD Single Cab Pick-up vehicle for the purpose of collecting and transporting locally generated non-biodegradable waste at Phedikhola Rural Municipality, Phedikhola Bazaar Headquarters in the current financial year 082/083, subject to the specified conditions, for a maximum of 21 working days per month until the end of Kartik 2082, and therefore this notice has been published for interested firms, individuals or companies to participate in the procurement process as per the rules. Interested firms, individuals or companies can obtain the bid form from this office by submitting the following documents and cash Rs. 1000/- along with a bank voucher or cash receipt for the bid fee paid within the office hours of 2082/05/04. The bidders should submit their sealed bids by 12.00 noon on 2082/05/05. No action will be taken against bids received after the deadline and not submitted in the prescribed manner. While submitting the bid, the vehicle owner should clearly mention the monthly rental rate of the vehicle he wants to provide in figures and letters in the bid form, sign and seal it. The following documents should be submitted along with the bid form. If the documents mentioned to be submitted are not submitted, such proposals will be rejected. The submitted bids will be opened at 13.00 noon on 2082/05/05 in this office. The following:

Documents to be submitted by individuals and firms along with the bid:

1. In the case of individuals, the bid must be submitted along with the following documents:

a) Rate submission form issued by this office (must be filled in and signed by the passenger or his/her designated representative)

b) A copy of the vehicle owner's Nepali citizenship certificate (both the front and back of the certificate)

c) A copy of the driver's Nepali citizenship certificate (both the front and back of the certificate)

d) Copy of the driver's driving license (renewable and showing both the front and back of the certificate)

e) Copy of the vehicle's blue book (must be submitted clearly showing the vehicle number, owner's name, vehicle type, vehicle details, and renewal page)

f) Copy of the vehicle's third party insurance document (including all pages issued by the insurance company)

2. In case of a company or firm, the following additional documents must be submitted along with the above-mentioned documents:

a) Copy of company or firm registration certificate

b) Copy of tax payment certificate up to FY 2080/81

c) Copy of the certificate of registration in the M.A.C. or PAN
