Documents and conditions to be attached:

1. Copy of the firm/company registration certificate from the Inland Revenue Office.

2. Copy of the certificate of registration and renewal with the Department of Medicines.

3. Tax payment certificate for the fiscal year 2080/81.

4. Certificate of Authorization in the case of an authorized seller.

5. Copy of the proprietor's citizenship.

6. The bid can be purchased from the administration branch of this office by paying Rs. 5,000.00 in cash within 30 days from the date of publication of the notice, i.e. 2082.05.22 by 5:00 PM.

7. The sealed bids received for registration will be opened on Bhadra 23, 2082 at 2:00 PM or, if that day falls on a holiday, on the next day, i.e. the day the office is opened, at the same time at Shivaganga Eye Hospital in the presence of the office representative and the bidder or his representative. The absence of the bidder or his representative will not prevent the opening of the bids.

8. The mentioned deposit amount should be deposited in cash in the account number of Nepal Red Cross Society Shiv Ganga Eye Hospital located at Prabhu Bank, Gulariya, of this branch. 00800124322000015, along with a bank guarantee letter (bid bond with a validity period of at least 120 days from the date of submission of the bid) issued in the name of this hospital from a recognized bank of Class A.

9. The bidder must submit a written self-declaration stating that he/she is not disqualified from participating in the relevant work, has no conflict of interest, and has not been convicted of any business-related offense.

10. Sealed bid forms purchased in the name of one firm or company cannot be submitted in the name of another firm or company.

11. No action will be taken against sealed bids that do not include the documents mentioned in this notice, are not filled in accordance with the specification rules, are not in order, have exceeded the deadline, or contain any conditions.

12. Other matters not mentioned in this notice shall be in accordance with the Public Procurement Act, 2063 and Regulations, 2064 (with amendments) and the Procurement Regulations of the Nepal Red Cross Society and other prevailing laws.

13. The sole authority to approve, reject, and cancel this procurement process shall be vested in the Procurement and Quality Assurance Committee of this District Branch.
