1. Bid Form The bidder or his authorized representative may purchase the bid form from Shree Krishna Netralaya Pvt. Ltd. Bhairahawa or Butwal branch within the 15th day from the date of first publication of the notice in Mechikali Rastriya Dainik by submitting a receipt of Rs. 500.00 (non-refundable), original voucher application along with the amount deposited, Value Added Tax Registration Certificate, copy of the settlement certificate up to FY 2080081, and a certified written application of self-declaration that he is not blacklisted.

2. If the last day for opening and submitting bids falls on a holiday, the next day when the office is open will be considered the last day.

3. The duly filled bid should be registered at Shree Krishna Netralaya Pvt. Ltd., Bhairahawa by 12:00 PM on the 16th day from the date of first publication.

4. The registered bids will be opened at 2.00 PM on the 16th day in the presence of the bidders or their representatives at Shree Krishna Netralaya Pvt. Ltd. Bhairahawa. However, there will be no impediment to the opening of the bids even if the bidders or their representatives are not present.

5. The rate quoted by the bidder should be clearly stated in figures and words without any corrections. In case of any difference in figures and words, the rate written in words will be accepted. If any corrections or additions are required, the signature of the authorized person and the firm seal should be affixed on the right and left sides of the cut and added places in a way that is understandable.

6. All taxes shall be borne by the bidder. No reimbursement of any taxes payable to the Government of Nepal will be made by this hospital.

7. Each bidder must fully comply with the conditions mentioned in the bid form and must mandatorily submit the evidence and details requested to be provided.

8. No action will be taken against bids received after the due date or after the deadline. If any other matters related to the bid are required, they can be understood at Shree Krishna Netralaya Pvt. Ltd. Bhairahawa during office hours.

9. The decision to accept, partially accept or completely reject the bid received, keeping in mind the interests of Shri Krishna Netralaya Pvt. Ltd., shall be vested in Shri Krishna Netralaya Pvt. Ltd. Bhairahawa.

10. The spectacle frames and lenses mentioned in the bid must be in accordance with the specifications requested by this hospital as mentioned in the bid.

11. The estimated quantity of medicines, surgical, lab supplies, lab spectacle frames and glass related items mentioned in the bid letter may increase or decrease as per the requirement and the mentioned items required by the hospital will be demanded repeatedly throughout the financial year.

12. When mentioning the price of spectacle frames and glassware, the price including VAT must be given for items subject to Value Added Tax.

13. An agreement will be signed between the suppliers selected for supply and the hospital within 7 days of receiving the letter of intent at the hospital.
